      What to Expect (Job Responsibilities):
      - Conduct in-home health evaluations, including medication and medical history reviews, and physical evaluations
      - Engage with patients to answer health questions and provide personalized care recommendations
      - Utilize technology (iPad and clinical workflows) to facilitate patient assessments
      - Travel locally within licensed states for patient visits and conduct virtual visits as needed
      - Provide diagnostic and preventive services as indicated by patient needs

      What is Required (Qualifications):
      - Active, unrestricted license(s) in coverage area(s)
      - Board certification as a Nurse Practitioner or Physician Assistant
      - Ability to cover primary locations, including Queens and surrounding areas
      - Willingness to travel locally and adapt to diverse home environments
      - Strong interpersonal skills to engage effectively with patients

      How to Stand Out (Preferred Qualifications):
      - Multi-state licenses in addition to primary state licensure
      - Previous experience in home health evaluations or similar clinical settings
      - Familiarity with diagnostic and preventive services such as spirometry and bone density screening
      - Ability to work independently and manage time effectively in a home setting
      - Commitment to improving health outcomes in the community
