      GENERAL SUMMARY/ OVERVIEW STATEMENT:

      IACs serves a key role in the Department of Medicine’s inpatient daily clinical operations. IACs primarily assist with admission and discharge planning. He/she will work primarily in conjunction with advanced practice providers (APPs), doctors, and residents of the inpatient general medicine, oncology, and cardiology service lines. The IAC assists with the coordination of patient care, with an emphasis placed on efficient and exemplary service in a high-volume inpatient setting. The IAC is expected to effectively organize priorities, complete tasks proactively, and manage confidential patient information. The IAC role helps to reduce administrative burden on the medical teams, expedites discharges and improves the coordination of post discharge care.

      PRINCIPAL DUTIES AND RESPONSIBILITIES:

      • Assist care team with patient throughput
      • IACs are assigned to clinical teams across the Department of Medicine, including General Medicine, Cardiology and Oncology.
      • Request medical records (Imaging discs, pathology slides, cardiac echoes, etc.) from outside hospitals (OSH).
      • Obtain and upload imaging from outside facility.
      • Submit/coordinate medication prior authorizations
      • Attend interdisciplinary team rounds for respective team (when feasible).
      • Proactively check-in with responding clinicians (Residents or APPs) daily.
      • Schedule infusions, transfusions, and office visits in appropriate timeframe prior to discharge.
      • Support safe transitions from inpatient to outpatient care
      • Coordinate post discharge follow-up appointments.
      • Faxes any inpatient notes/discharge summaries to PCPs/ specialty offices.
      • Develop a basic knowledge of public and private insurance coverage for inpatient and ambulatory services and medications.
      • Review discharge medication list and initiate prior authorization process for medications to ensure completion prior to discharge.
      • Facilitate specialty pharmacy requests and arrange for timely delivery of medications.
      • Coordinate with patients and caregivers the delivery of medications and supplies prior to discharge.
      • Troubleshoot within a complex system to resolve scheduling issues and escalate concerns about gaps in follow up.
      • Communicate with patients, families, and case management to troubleshoot issues with co-pays, medication deliveries, and transportation.
      • Provide cross coverage for other team members as needed.
      • Maintain confidentiality and privacy consistent with HIPAA (Health Insurance Portability and Accountability) guidelines.
      • Develop and maintain relationships with the Physicians, Residents, APPs, Operation Associates, Nurses, Case Managers and members of care teams and units
      • Other administrative tasks as assigned.

      WORKING CONDITIONS:

      · Administrative office and hospital setting.

      · This position requires the incumbent to walk, from the administrative office to the hospital’s main campus multiple times a week.

      · This is an in-person position, Monday – Friday.

      Qualifications

      SKILLS/ABILITIES/COMPETENCIES REQUIRED:

      · Strong communicative, interpersonal and team skills

      · Computer savvy with strong ability to learn and navigate multiple systems

      · Knowledge of HIPAA Confidentiality and Privacy Policies

      · Demonstrates a willingness to learn

      · Identifies trends and opportunities for improvement

      · Time management and ability to independently prioritize work

      EDUCATION:

      • Associates or bachelor's degree preferred.
      • Minimum of high school diploma or equivalent required.

      EXPERIENCE:

      • 1-2 years' experience in healthcare preferred or new graduate with demonstrated interest in healthcare.
      • Prior experience with Prior Authorizations and specialty medications preferred.
      • EPIC Medical Record experience preferred.
      • Demonstrated ability to learn and understand complex systems.
