Department ProfileThe Case Management Department is an integrated model with registered nurse and social work case managers. The case managers work together to assess patient and family needs across the hospital. They work collaboratively with all disciplines, patients, and families to achieve mutually agreed upon care goals. Case managers facilitate multi-disciplinary rounds to effectively transition patients safely to the next level of care and manage throughput initiatives. Our emergency department is staffed 24/7 with case managers and we have a dedicated observation management team.Job SummaryUnder general supervision of the Manager, the Team Lead will support the clinical expertise of the Case Management Department. Accountable for coordinating departmental activities including daily staffing, scheduling, and process workflow to ensure team members follow established policies, procedures, and professional guidelines. Arranges for staff coverage when indicated and provides coverage as necessary to patient needs. Provides side-by-side support, mentorship, and skill development to all team members. The team lead promotes collaboration among departmental disciplines and interdisciplinary partners. Identifies strengths and opportunities in clinical practice, capturing themes for process improvement to escalate to leadership. Strong problem-solving required for effective real-time intervention. Responsible for ensuring that all staff perform job functions in accordance with mission, vision, and values of Tampa General Hospital.Experience: Three (3) years clinical experience with one (1) year of experience in Case Management or two years in Emergency Management.
Education: Graduate of an accredited school of Nursing. Associate degree required. Licensed to practice as a Registered Nurse by the State of Florida
OR Master of Social Work degree from a C.S.W.E.-accredited college or university Social Work program. Florida Licensed Social Worker (LCSW) or completion of FL Licensed Social Worker (LCSW) within sixty days. Ability to read, write and speak English clearly and distinctly. Knowledge of Microsoft Office including Word, Excel, and PowerPoint required. Knowledge of Microsoft Access, Project, Publisher, and MS Vizio, Microsoft Team Presenting platform, audio/visual hardware desired.
