What to Expect (Job Responsibilities):
- Conduct initial and follow-up psycho-social assessments and social service intakes
- Identify patients' risk factors and develop appropriate care plans
- Provide follow-up and evaluation of high-risk patients, intervening in cases of violence, abuse, depression, and substance abuse
- Educate patients and their families about available community resources
- Provide direct social services and case management assessments

What is Required (Qualifications):
- LCSW licensure
- Experience in conducting psycho-social assessments and social service intakes
- Knowledge of community resources and ability to provide appropriate referrals
- Strong counseling and crisis intervention skills
- Excellent communication and collaboration abilities
