      Short Description

      The Lead Population Health Advisor will serve as a team leader, resource, preceptor, and mentor to the health advisors for the department of population health, innovative delivery. They will have ownership of the development of the orientation process and workflow for new hires and responsible for continued management of the orientation workflow/process. They will serve as a resource to the department, patients, families, and health care team members within the scope of their role. They will collaborate with the department managers on areas of workflow opportunities within the health advisor team as needed.

      Develops and Facilitates new hire orientation, training, speaker opportunities for population health team development.

      Leads team huddles as designated, supports development of team-based education and improvement projects, serves as resource, and mentor.

      Supports the development and maintenance of education materials, workflows, policies, and procedures for the health advisors as designated.

      Represents Health Advisor Team at a departmental and divisional level meetings as designated.

      Expected to maintain behavior, judgment and attitude which reflect professional practice standards.

      Expected to continue to perform roles and responsibilities as outlined in the Health Advisor Job Description
