What to Expect (Job Responsibilities):
- Ensure seamless transitions for CCBHC patients across the health services spectrum
- Collaborate with clinical and non-clinical staff, external providers, and community resources
- Assist clients in overcoming barriers to care and improving health outcomes
- Maintain accurate and timely documentation of care coordination in the electronic health record
- Manage patient electronic health records and prioritize multiple tasks effectively
What is Required (Qualifications):
- Alabama Licensed Practical Nurse (LPN) in good standing
- Minimum of three years of experience in nursing
- Experience working with and accessing community resources
- Proficiency in accurate and timely documentation of care coordination
- Demonstrated ability to work with multi-disciplinary teams
How to Stand Out (Preferred Qualifications):
- Experience working with special populations as required by CCBHC
