What to Expect (Job Responsibilities):
- Develop, assess, and coordinate holistic care management activities for long-term care members
- Create personalized service care plans and educate members and their families on available services
- Monitor care plans and member status, providing recommendations based on identified needs
- Facilitate communication between members, families, and care providers to ensure access to services
- Conduct on-site visits to assess member needs and collaborate with healthcare providers
What is Required (Qualifications):
- Bachelor's degree and 1 year of related experience
- For Illinois Plan: RN licensed in Illinois, or a Bachelor's/Master's degree in a human services-related field, or LPN with 1 year of relevant experience
- Strong understanding of long-term care services and community resources
- Excellent communication and interpersonal skills
- Ability to comply with state, federal, and third-party payer regulations
How to Stand Out (Preferred Qualifications):
- Experience in case management, advocacy, or conducting home visits
- Familiarity with the needs of physically disabled or elderly individuals
- Skills in developing care plans and coordinating services
- Background in healthcare or social work
- Knowledge of compliance standards in healthcare settings
