What to Expect (Job Responsibilities):
- Evaluate member needs and recommend plans for optimal healthcare outcomes
- Develop and coordinate long-term care plans/service plans for members
- Facilitate communication between members, families, and care provider teams
- Monitor and document member status and care management activities
- Provide education to members and families on healthcare services and benefits
What is Required (Qualifications):
- Bachelor’s degree and 1 year of related experience
- For Illinois Plan: RN licensed in Illinois, or a Bachelor's/Master’s degree in a human services-related field, or LPN with one year of experience in comprehensive assessments for the elderly
- Ability to assess member needs and coordinate care effectively
- Strong documentation skills to ensure compliance with regulations
- Excellent communication skills for interacting with members and healthcare providers
How to Stand Out (Preferred Qualifications):
- Experience in long-term care management or related healthcare services
- Knowledge of Medicaid and healthcare regulations
- Familiarity with community resources for long-term care members
- Strong problem-solving skills and ability to work independently
