      What to Expect (Job Responsibilities):
      - Assist in developing, assessing, and coordinating holistic care management activities for long-term care members
      - Evaluate member needs and recommend or facilitate plans for optimal healthcare outcomes
      - Coordinate between members, families, and care providers to ensure accessibility to services
      - Monitor care plans and member status, providing recommendations based on identified needs
      - Conduct on-site visits to assess member needs and collaborate with providers or resources

      What is Required (Qualifications):
      - Bachelor’s degree and 1 year of related experience
      - For Illinois applicants: Must meet one of the following criteria:
      - RN licensed in Illinois
      - Bachelor or Master’s degree in a human services-related field
      - LPN with one year of experience in conducting comprehensive assessments for the elderly
      - Ability to interact effectively with long-term care healthcare providers and partners
      - Strong documentation skills for maintaining member information and care management activities
      - Knowledge of state, federal, and third-party payer regulations

      How to Stand Out (Preferred Qualifications):
      - Experience in case management or care coordination within a healthcare setting
      - Familiarity with long-term care services and resources
      - Strong communication skills, both written and verbal
      - Experience working with elderly or physically disabled individuals
      - Knowledge of community resources for healthcare and social services
