      What to Expect (Job Responsibilities):
      - Assist in developing and coordinating holistic care management activities for long-term care members
      - Evaluate member needs and facilitate personalized service care plans
      - Coordinate between members, families, and care provider teams to ensure accessibility of services
      - Monitor care plans and provide recommendations based on member status and needs
      - Collect and maintain member information to ensure compliance with regulations

      What is Required (Qualifications):
      - Bachelor's degree and 1 year of related experience, or equivalent experience
      - For Illinois Plan Only: RN licensed in Illinois, or a Bachelor/Master’s degree in a human services-related field, or LPN with 1 year of experience in assessments for the elderly
      - Must reside within specified Cook County ZIP codes
      - Availability for a Monday to Friday work schedule, 8 AM - 5 PM during training and 8 AM - 4:30 PM after training
      - Ability to perform on-site visits to assess member needs

      - Experience in care management or working with long-term care populations
      - Knowledge of community resources and healthcare services
      - Strong communication and interpersonal skills
      - Ability to work independently and as part of a team
      - Familiarity with state and federal healthcare regulations
