      What to Expect (Job Responsibilities):
      - Develop, assess, and coordinate holistic care management activities for elderly and physically disabled members
      - Assist in creating personalized service plans and educate members and their families on available services
      - Monitor care plans and member status, providing recommendations based on identified needs
      - Perform on-site visits to assess members' needs and collaborate with care providers
      - Collect and maintain member information to ensure compliance with regulations

      What is Required (Qualifications):
      - Bachelor's degree and 1 year of related experience
      - For Illinois Plan: RN licensed in Illinois, or a Bachelor's/Master's degree in a human services-related field, or LPN with 1 year of experience with elderly assessments
      - Strong understanding of long-term care services and resources
      - Excellent communication and interpersonal skills
      - Ability to work independently and manage caseload effectively

      How to Stand Out (Preferred Qualifications):
      - Experience in case management or working with aging and disability waiver members
      - Familiarity with community resources available for long-term care members
      - Knowledge of state, federal, and third-party payer regulations
      - Experience in providing education and support to families and caregivers
      - Ability to facilitate collaboration among various healthcare providers
