      What to Expect (Job Responsibilities):
      - Collaborate with referral sources and sales teams to gather necessary information for processing quality referrals
      - Obtain and document verbal orders for patient evaluations as required by state regulations
      - Accurately document all referral-related information and follow-up actions in the medical record
      - Ensure timely attention to referrals received from various platforms, including eFax and hospital EMR portals
      - Review eligibility alerts and responses for Medicare clients, as applicable

      What is Required (Qualifications):
      - Current LVN License required
      - 3-7 years of experience in facility/physician relationships with knowledge of facility discharge processes
      - Minimum of one year of experience in home care, hospice, or personal care services
      - Familiarity with Electronic Medical Records (EMR)
      - Experience dealing with various healthcare payors, including Medicare and Medicaid

      How to Stand Out (Preferred Qualifications):
      - Additional experience in home health care settings
      - Knowledge of ICD-10 coding processes
      - Strong communication skills and ability to work collaboratively in a team environment
      - Familiarity with healthcare software solutions
      - Previous experience in a fast-paced healthcare environment
