      Base Rate: $103,350 - $155,000 (salary band placement to commensurate with experience)

      Position Overview

      Reporting directly to the Program Director, Palliative, Seniors’ Services, Primary Care and Health System Integration, and with accountability to Program Chief and Medical Director, this position brings strong leadership and business management skills to teams working to build and sustain excellence at Trillium Health Partners. The Manager, Business Operation is an integral member of the Palliative, Seniors’ Services, Primary Care and Health System Integration Program’s leadership team.

      Working in collaboration with the Manager, Primary and Integrated Care, this position is responsible for the development and management of THP programs and services that deliver on our strategic direction of integrated health care. These innovative programs facilitate care across the continuum in partnership with other organizations and providers in the community. Programs presently include THP@home and bundled care and will be responsible for other integrated care initiatives as they are identified.

      The successful candidate has a demonstrated track record of engaging and motivating multi-disciplinary clinical and enabling teams to provide high quality business and services development and sustainment, rooted in improving patient and family centered care. They will have experience in building programs with external stakeholders against a shared vision.

      The preferred candidate has proven and effective leadership and business management skills including change management, project management, systems design and analytics, contract management, a commitment to high quality work, patient safety, and best possible outcomes, a strong customer service orientation and a strong work ethic grounded in integrity and values that are aligned with Trillium Health Partners.

      Job Description:

      • Work collaboratively with Program Leadership to facilitate and develop program vision, workforce development, deliverables and work plans.
      • Maintain a summary of best-practice evidence, and promote knowledge acquisition for team members
      • Achieving the highest quality of patient-centered care and exceptional experience that aligns with THP strategic goals of quality, access, and sustainability.
      • Work within the Integrated Care Office, an interdisciplinary, cross functional team, to support the organization’s strategic priority of developing Integrated Care Pathways and Systems to support care beyond the walls of the hospital.
      • Critical and strategic thinking to lead complex cultural change in a fast-aced environment and deliver sustainable effective and efficient results.
      • Supporting operational effectiveness within a diverse clinical team to maximize resource utilization.
      • Work collaboratively with Manager, Primary and Integrated Care to identify and advance change initiatives to promote excellence in service delivery, partnership development and operational effectiveness.
      • Supporting the various programs to meet reporting requirements, including working with Decision Support, Clinical Informatics and Business Intelligence to develop and sustain robust reporting and analytics infrastructure which both evaluates programming and anticipates community needs.
      • Facilitate Request for Proposal (RFP) processes, and contract writing with providers and vendors.
      • Lead and design partner invoicing processes, reporting and performance management.
      • Work with Finance team to monitor and evaluate the financial sustainability of programming and operations.
      • Monitor variances related volumes, utilization, etc. to identify and implement proactive process improvement opportunities.
      • Responsible for ongoing reporting requirements against program performance.
      • Responsible for administration of electronic interfaces with THP health record (i.e. including but not limited to CHRIS); as well as product launch and maintenance when required for operational innovation, business and/or reporting needs

      Leadership Responsibilities:

      • Review, analyze and apply data to support decision-making and continuously improve quality as well as innovative planning to achieve performance targets.
      • Lead process improvement initiatives within the team and organization to achieve long-term success.
      • Lead the development of new integrated care programs through co-design with clinical partners, patients and families, and community care providers; this development will include proposal development, budgeting, change management, partner engagement, etc.
      • Collaborate with Primary and Integrated Care clinical managers and physician leads to monitor and optimize operations.
      • Support governance structures for oversight of primary and integrated care initiatives.
      • Lead and manage direct reports as assigned in future.
      • Actively participates in committees, both internal and external, to promote collaboration and coordination of services with all stakeholders and partners.
      • Develop flexible, responsive system to meet changing demands.

      Qualifications:

      • Baccalaureate degree in Business, Health Focus, or other relevant training
      • Master’s degree in business administration, health administration or current registration in a Graduate program preferred.
      • PMP designation strongly preferred.
      • Minimum five years of progressive management/leadership experience in business or clinical operations. Experience with and knowledge of provincial health and community service systems an asset.
      • Lean certification or equivalent process improvement methodology is preferred.
      • Record of accomplishment in innovative and successful program development preferred.
      • Ability to navigate ambiguity in a complex and fast-paced environment.
      • Proven success in managing change and transforming workplace culture with the ability to establish positive relationships.
      • Proven ability to successfully manage multiple and competing priorities.
      • Effective team building and creative thinking skills.
      • Excellent organizational, communication, interpersonal, problem-solving, negotiation, conflict resolution and decision-making skills are essential to collaborate with highly skilled staff such as physicians, nurses and students and external partnerships.
      • Advanced working knowledge of computers and relevant software with solid knowledge and experience of working with large databases to extract reports and understand trends.
      • Excellent performance and attendance record required.
