What to Expect (Job Responsibilities):
- Conduct telephonic outreach to patients post-discharge from Emergency Room or Inpatient setting
- Interview patients to assess treatment needs and update patient information
- Verify hospital discharge information and advocate for needed services
- Assist Case Managers with discharge and post-discharge coordination
- Maintain patient records and log events and progress

What is Required (Qualifications):
- Medical Assistant Certification/Licensure
- Minimum of two (2) years of experience as a Medical Assistant or in patient healthcare in a clinical setting
- Knowledge of medical practice/terminology and care
- Strong organizational and interpersonal skills
- Bilingual (fluent in English and Spanish preferred)
