Job Description
- Req#: 534411
- Review clinical and physician documentation daily and quarterly as directed
- Ensure proper support for ICD-10 and CPT codes in documentation
- Review documentation based on LCD/NCD and NCCI edits per region
- Evaluate chief complaint, physical exam, HPI, ROS, history, medications, allergies, procedures, and clinical findings
- Communicate with clinicians to clarify or query documentation as needed
- Accurately enter review findings into Luvo® Lighthouse and submit reports within required timeframes
- Maintain up-to-date knowledge of regional LCD/NCD and NCCI edits
- Collaborate with regional team members on documentation concerns and improvements
- Maintain confidentiality and compliance with HIPAA guidelines
- Perform additional related duties as assigned
- High school diploma or equivalent required
- Previous Clinical Documentation Improvement (CDI) experience is preferred but not required.
- RN license encouraged, but not required.
- Ability to multi-task and communicate consistently in a professional manner
- Knowledge of billing and coding preferred
- Understanding of medical terminology used in clinical settings
- Ability to read and review medical charts in an electronic health record (EHR) system
- Adequate computer skills and comfort viewing digital content for extended periods of time

Please Note: This position requires that you reside in the United States and be legally eligible to work in the U.S. Being located in the Los Angeles area is a plus, but not required.
This is a full-time, Monday through Friday position, scheduled for 40 hours per week.
JOB SUMMARY:
Under the supervision of the Director of Documentation Integrity, the Full Time Clinical Documentation Review Specialist/Medical Document Quality Reviewer is responsible for reviewing clinical documentation for completeness, accuracy, and compliance. You’ll conduct both concurrent and retrospective reviews, communicate with clinical teams to clarify documentation, and ensure all records meet coding and regulatory standards.
