Ensures safety, confidentiality, regulatory compliance, and adherence to hospital and infection control standards.
Coordinates interdisciplinary care, advocates for patients and families, and oversees safe, comprehensive discharge planning.
Provides behavioral health case management, assessments, therapy, crisis intervention, and ongoing treatment planning with accurate documentation.
Communicates effectively with patients, families, and community partners while maintaining strong clinical, organizational, and technology skills.
Education: Master's degree required in Social Work, Counseling or other human-service related field.
Licensure: Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), or Licensed Professional Counselor (LPC) in Missouri required. Only a Licensed Clinical Social Worker (LCSW) is accepted at Mercy Hospital Perry.
Experience: Minimum 2-3 years Behavioral Health experience preferred.
Preferred Other: Co-worker must be able to communicate orally, in writing and demonstrate proficient computer skills. Co-worker must be able to listen and to understand and explain complex situations.
Hours: Monday-Friday 8am-4:30pm
