Nurse Coach
Job Description
Join our team as a Nurse Coach, where you will use your clinical expertise to manage Post-Acute Care patients’ transition from acute care settings to home through telephonic outreach. This involves providing education, coaching, and care coordination to accomplish the goals of the Post-Acute Care Program. This is a telehealth role, so you will be on the phone for the majority of your 8-hour shift. Your responsibilities include engaging patients, administering assessments, working closely with home health agency nurses and physicians, reviewing utilization information, documenting all interactions, and participating in ongoing quality assessment/improvement activities. This role requires Evening and Weekend coverage.

Work Site
This is a fully remote position.
Work Environment
This is a 100% remote position, with all necessary equipment provided. Training is 4-6 weeks from Monday to Friday, 8am-5pm EST. After training, the schedule is Monday to Wednesday and Friday to Saturday, 9am to 6pm EST. You will have Thursdays and Sundays off. Expect a 1-hour lunch break and two 15-minute breaks. No time off will be granted for the initial 90 days on-contract. The role involves use of an auto-dialer, resembling a call center environment. Comfort with sitting/working at a computer for 8 hours/day is a must.
