      What to Expect (Job Responsibilities):
      - Conduct in-home assessments on health plan members, including comprehensive history, physical exams, and medication reviews
      - Engage in end-of-life care discussions with patients and their families
      - Collect laboratory specimens in the home setting when appropriate
      - Identify diagnoses for active medical management and chronic disease management
      - Communicate assessment findings to primary care providers to address potential gaps in care

      What is Required (Qualifications):
      - Master's degree from an accredited Nurse Practitioner or Physician Assistant program
      - Active California state Nurse Practitioner or Physician Assistant license
      - Active RN License and Furnishing number (for Nurse Practitioners upon hire)
      - Active Nurse Practitioner Board Certification or Physician Assistant Certification
      - Valid California driver’s license and current automobile insurance

      How to Stand Out (Preferred Qualifications):
      - One (1) year of prior clinical or home care experience, preferably with older adult (geriatric) patients
      - Previous experience with electronic medical records (EMR)
      - Strong problem-solving, organizational, and time management skills
      - Ability to communicate positively and effectively with others
      - Familiarity with community resources for patient referrals
