- A dedicated patient base and a consistent referral flow, eliminating the need to worry about referral volume.
- Collaborate with the skilled nursing facility’s integrated care team along with Vitae’s clinical team including adult nurse practitioners, psychiatric nurse practitioners, psychologists, LCSWs, and podiatrists.
- A balanced work-life schedule with flexible hours. No nights, weekends, or off-hours are required.
- Malpractice insurance coverage of $1 million/$3 million, provided by Vitae.

Founded in 2017, Vitae Health Systems is one of the largest providers of internal medicine and behavioral health services to the residents of Skilled Nursing and Assisted Living Facilities. Headquartered in Chicago, IL, we currently operate in over 10 states and are rapidly expanding throughout the USA.
The Position
As we continue to grow, we are looking for full-time nurse practitioners to provide care for the geriatric population in a Skilled Nursing and/or Assisted Living setting. Our nurse practitioner positions can include:
