      What to Expect (Job Responsibilities):
      - Conduct comprehensive in-home history and physical assessments to identify active and chronic health conditions
      - Capture and document annual diagnoses accurately for Medicare risk adjustment
      - Collaborate with care team members to develop and implement medical plans of care
      - Educate and counsel members and their families on identified health conditions
      - Ensure timely appointments and documentation in accordance with departmental procedures

      What is Required (Qualifications):
      - Must be a Nurse Practitioner or Physician Assistant with appropriate education and licensure
      - Proficient in MS Office products, including Excel, Word, Outlook, and PowerPoint
      - Strong organizational skills with the ability to multitask and prioritize projects
      - Excellent communication skills to effectively interact with a diverse population
      - Current national certification as a Nurse Practitioner or State certification as a Physician Assistant, along with an unrestricted driver's license

      How to Stand Out (Preferred Qualifications):
      - Previous experience as a Registered Nurse, Nurse Practitioner, or Physician Assistant, particularly with geriatric or disabled populations
      - Two or more years of clinical experience in a relevant field
