- To provide specialist Occupational Therapy assessment, treatment and review of patients with a wide variety of conditions including neurological, orthopaedic, musculoskeletal, care of the elderly, respiratory and falls, to reduce unnecessary hospital admissions, contribute to effective admission avoidance and support patients with complex health care needs. This will be for patients predominantly in their place of residence, but will include short term placements and other settings.
- To provide advice and guidance to others, patients, carers and staff working in health and social care.
- To support professional development of the staff within One Coventry Integrated Team and other services as appropriate.
- To support and contribute toward service development and meeting Key Performance Indicators.
- To participate in 7 day working rotas to ensure delivery of key services.
- To be an integral part of One Coventry Integrated Team (OCIT)
- To effectively manage a caseload of patients as part of a multidisciplinary team (MDT), providing specialist and evidence-based assessment and interventions to patients in both a stable and acute phase of their condition, ensuring the physical, psychosocial, behavioural, perceptual, cognitive, sensory and environmental needs of patients, are addressed.
- Utilising specialist knowledge and clinical reasoning, to plan and provide clinical interventions in consultation with other colleagues in the multidisciplinary team and to work in a range of settings as appropriate (e.g. patients home, short term placement or other community setting)
- To adopt a patient centred and sensitive approach to therapy, collaborating with patients (and/or carer(s) where appropriate) in all aspects of the therapy process (assessment, planning, intervention, evaluation, reporting).
- To utilise individual and/or group work skills as appropriate to achieve agreed therapeutic goals
- To have a high level of knowledge of a variety of conditions including neurological, orthopaedic, cognitive impairment, care of the elderly, respiratory and falls and specialist knowledge of at least one of these clinical areas
- To work collaboratively with Social Services and third sector providers to ensure delivery of effective safe care.
- To complete holistic assessments of patient needs with a focus on enablement, home first and effective utilisation of resources
- To take a lead for information management to ensure high quality data, which is recorded, evaluated & reported in a timely manner.
- To work collaboratively with the team leaders and LIT manager to participate in evaluation of the OCIT service and identify opportunities for improvement and service development.

Job summary
An exciting opportunity has arisen for Occupational Therapists to join the new and transformational One Coventry Integrated Team (OCIT) service. The trust along with Coventry City Council and Coventry and Warwickshire Partnership Trust have designed and developed 3 Local Integrated Teams (LITs) across the city of Coventry which are supported by multi-professional teams offering a holistic and integrated approach to urgent community needs for the residents of Coventry.
Local integrated care is one of the Trust's fundamental purposes, as set out in our refreshed strategy for 2022-2030; 'More than a hospital'. The Trust has an ambition to deliver improvements in the health and wellbeing of our local populations through collaboration with partners across the health and social care system. The role of Occupational Therapist is key to the successful delivery of this exciting and transformational service aimed to improve the health and wellbeing of the Coventry population.
