      • Coordinates the organ allocation process for prospective candidates in a timely manner; evaluates existing organ offers and maintains medical records of candidates, ensuring patient files are complete and databases (e.g., UNET) are updated, as necessary.
      • Educates patients, family members about the organ offer process; answers appropriate questions as necessary.
      • Identify, document, and communicate required clinical information to Donor Network team members and transplant center staff.
      • Relates patient concerns and confers with transplant team during organ offer and allocation process, in addition to communication between transplant physicians and patient.
      • Coordinates transplantation process when a donor is available (24 hours/7 days a week) including communication between physicians, local organ procurement organization, patient/families and the medical team involved including ED, OR, ICU (SICU), blood bank and histocompatilbility labs
      • Coordinates multiple levels of the allocation of organs for transplant or research according to established Northwell Health and OPTN policies.
      • Provides assistance during local donor identification, stabilization and management of organ donors.
      • Maintains logistical oversight including transport of organs for transplant or research that are awaiting final disposition.
      • Relays accurate and timely donor medical information from the Northwell Health EMR systems or from the UNET Donor Net system to transplant surgeons and transplant center personnel to facilitate organ placement.
      • Collaborates with the surgical teams responsible for recovering the organ(s) through effective communication to facilitate the donation process. Schedule offsite recovery facilities, arrange travel arrangements, arrange donor transport, and coordinate logistics with other transplant agencies.
      • Devise a list of matching recipients once a donor has been identified
      • Ensure organs remain viable for donation and make sure they are delivered on time to the recipient
      • Participates in the development and implementation of protocol and standards of care for the transplant patient in collaboration with the multidisciplinary team.
      • Serves as the subject matter expert in organ allocation and associated policies.
      • Assesses data reflective of the patient's status and appropriately interprets information relative to the patient's age-specific needs.
      • Performs related duties as required.

      EXPERIENCE & QUALIFICATIONS - REQUIRED:

      • Associate of Arts or Science degree required AND two (2) or more years’ experience in OPO, transplant, critical care, or training to include EMT, paramedic and/or other related experience.
      • Availability to work 12-24 hour shifts.

      EXPERIENCE & QUALIFICATIONS - PREFERRED:

      • Bachelor of Arts or Science degree
      • Sound knowledge of UNOS policy and CMS regulatory requirements in relation to organ allocation, waitlist management as well as ongoing patient status updating.
      • Demonstrates familiarity with complex logistical support and executes care coordination tasks in a timely manner.

      Current License to practice as a Registered Professional Nurse in New York State OR Current License to practice as a Physician Assistant in New York State OR Current License to practice as a Registered Respiratory Therapist in New York State
