  What to Expect (Job Responsibilities):
  - Conduct direct telephonic outreach to patients on a provided list
  - Assist patients in setting up appointments with the clinical care team and their primary care provider
  - Use motivational interviewing tactics to help patients overcome barriers to scheduling appointments
  - Track outreach efforts and document outcomes using the designated tracking system
  - Maintain a collaborative working relationship with the designated market team and practice leadership
  
  What is Required (Qualifications):
  - High School Diploma
  - Minimum of 2 years of healthcare experience
  - At least 1 year of telephonic customer service experience
  - Understanding of call center performance operations
  - Proficient with Microsoft and Google applications
  
  How to Stand Out (Preferred Qualifications):
  - Experience working in a medical practice or healthcare setting
  - Medical Terminology certification
  - Bilingual skills
  - Experience using electronic health record (EHR) platforms
