We have an exciting opportunity for a Palliative/End of Life Frailty Lead to join our team at St Lukes Hospice, working predominantly in the community setting.
This fabulous opportunity is to help support the delivery of outstanding quality care to frail and vulnerable people within Plymouth and surrounding areas, supporting patients with moderate to severe frailty and could be within the last year of life.
You will use your experience and specialist palliative care knowledge to work closely with health professionals to deliver appropriate management, advance care planning, undertake assessment and medication review for end-of-life care.
As part of our commitment to a continuous improvement strategy for this group of individuals and, in order to reduce variation in care with the focus being on standardising practice we have created this innovative post. This is a new role within St Lukes Hospice and will be pivotal to the delivery of a new pathway for the frail and elderly who are in their last year of life and approaching end of life. The successful candidate will ensure that the frail person is at the centre of decision making, ensuring that the risk of these patients being admitted to hospital unnecessarily is reduced.
A further aim of the role is to work towards a model that ensures those patients who require hospitalisation are admitted and discharge is proactively expediated to their preferred place of care.
