- Links patient and family to essential support services and other needs as identified, in collaboration with clinical staff. Addresses identified SDH needs and improves access to community resources.
- Develops and implements health education programs and materials. Collects and reports data on program activities.
- Establishes and maintains close relationships with housing resources, food pantries, utility companies, government agencies, treatment facilities and other community-based organizations.
- Assists in conducting follow-up calls to patients and families to confirm linkage with identified community resources or services, assess potential barriers, and aids in navigating resources and services. May accompany patients and families to social service appointments.
- Provides appropriate documentation of patient and family interactions in EPIC.
- Remains informed of trends, developments, as well as state-of-the-art prevention efforts and strategies, and general information regarding the targeted community.
- High School Graduate or equivalent, required.
- Associates Degree, preferred.
- Effective customer service, communication, and interpersonal skills required for interaction with patients, families, and community providers.
- Ability to work as a part of a team, with diverse populations, (i.e. racial/ethnic, socioeconomic and educational backgrounds), and with healthcare providers in clinical settings.
- Strong attention to detail and organizational abilities required. Sound decision making skills, with the ability to maintain highly confidential information.
- Moderate computer skills required (familiarity with Microsoft Word and Excel, preferable).
- Ability to travel and work flexible hours, including some evenings and weekends.
- One year of professional experience with a social service agency, community-based program, or health-related institution, required.
- Experience working or volunteering with children or families, required.

Licensure Requirement:Valid Ohio driver’s license and proof of auto insurance as required by hospital policy and position-specific requirements. Must pass motor vehicle background inspection, insurance eligibility, driving qualifications, and training set forth by Nationwide Children’s Hospital and maintain qualification of insurance guidelines.
Certifications:Community Health Worker certification by the Ohio Board of Nursing required or Completion of the Community Health Worker Training Program; certification must be obtained within one year of employment.
