      What to Expect (Job Responsibilities):
      - Outreach to patients using various communication platforms, including text messages, phone calls, and emails
      - Schedule and confirm appointments with patients while maintaining electronic medical records
      - Update and correct patient health records in the patient management system and electronic medical record
      - Provide portal support, including patient outreach and enabling access
      - Run regular and ad hoc reports related to patient engagement activities

      What is Required (Qualifications):
      - Minimum of an Associate's Degree or Medical Assistant Certificate with one year of experience
      - Strong customer/patient service experience
      - Computer literacy and proficiency in using related software applications
      - Excellent oral and written communication skills
      - Remote workers must have access to reliable high-speed Internet and reside in the United States

      How to Stand Out (Preferred Qualifications):
      - Bilingual in Spanish
      - Experience with patient engagement platforms such as Centricity and eClinicalWorks
