What to Expect (Job Responsibilities):
- Conduct outbound calls to eligible patients to facilitate in-home visit scheduling and re-engagement in the clinical program
- Utilize strong communication skills to understand patient needs and explain services clearly
- Research patient records to determine eligibility and past visit history
- Document call notes and scheduling details accurately in electronic systems
- Collaborate with team members to address patient concerns and enhance recovery processes
What is Required (Qualifications):
- High school diploma
- Ability to multi-task and prioritize competing tasks
- Excellent verbal and written communication skills
- Strong customer service skills with an emphasis on empathy and patient care
- Goal-driven with the ability to meet monthly scheduling targets
