      POSITION DESCRIPTION:

      The Population Health – Health Equity Specialist provides specialized project management in the strategic development, management, and maintenance of projects to address health equity and social care integration across Trinity Health IHA Medical Group. The Health Equity Specialist builds positive relationships with community-based organizations and actively participates on committees and health initiatives across Trinity Health and IHA. This position will be tasked with leading complex health equity and social care integration projects by directing multi-stakeholder teams from across IHA, to identify project goals, establish and manage timelines, direct workflow, and provide support to the team, as needed.

      ESSENTIAL JOB FUNCTIONS:

      1. Responsible for advancing organizational strategy of health equity and social care integration across IHA.
      2. Leads complex projects by directing multi-stakeholder teams from across IHA, to identify project goals and associated timelines, deliverables, milestones and appropriately escalates issues that may compromise project timelines, goals, or budget.
      3. Responsible for ensuring that IHA meets payer requirements for health equity and social care integration.
      4. Supports the development and implementation of programmatic processes to meet new payer requirements for health equity and social care integration.
      5. Manages financial opportunities and is accountable to a budget for health equity and social care integration including payer incentives, grant opportunities, etc.
      6. Responsible for the management and oversight of community resources including transportation resources (e.g. Lyft Contract), food resources (e.g. prescription for health), etc.
      7. Co-Chairs the IHA Diversity and Inclusion Advisory Council along with the Physician Champion for Diversity and Inclusion.
      8. Develops, manages and provides oversight of the health equity dashboard.
      9. Partners with Business Intelligence team and Utilization & Population Health Healthcare Data Analyst to develop analytics to identify areas of opportunity to reduce health disparities and promote social care integration.
      10. Proactively identifies trends and matches resources as they relate to health equity and social care integration.
      11. Stays up-to-date with the health system's strategic plan, the Community Health Needs Assessment (CHNA), and the accompanying CHNA Implementation Strategy.
      12. Compiles and delivers frequent progress reports to the Director of Quality and Population Health and other department leaders, as needed.
      13. Develops, implements and evaluates programs focused on health equity and meeting the identified needs of IHA’s patient population.
      14. Collaborates with Trinity Health and Local Health Department Community Health Worker programs.
      15. Builds Community Partnerships and actively participates on committees and health initiatives across the health system.
      16. Collaborates with Trinity Health System Office and FindHelp/Community Resource Directory to ensure resources are accurate and up to date.
      17. Communicates thoroughly and promptly with the Population Health and Quality Leadership team for potential issues and makes recommendations pertaining to new and ongoing projects.
      18. Analyzes, prepares, and presents graphical representations relating to project status and interprets findings.
      19. Provides direction, approval and/or project recommendations for new projects that are clear and well-structured. Identifies new project and existing project needs.
      20. Assists with special projects and performs other duties, as assigned.

      Performance that meets or exceeds IHA CARES Values expectation as outlined in IHA Performance Review document, relative to position.

      ESSENTIAL QUALIFICATIONS:

      EDUCATION: Bachelor’s degree from an accredited college/university in public health, social work or a health related field; master’s degree in public health, social work or health related field preferred.

      CREDENTIALS/LICENSURE: N/A

      MINIMUM EXPERIENCE: Minimum of two years’ experience managing complex projects on the strategic roadmap, with the ability to work independently and recognize opportunities for program development. Experience in working in a matrix-managed organization. One to three years of experience in public health, health promotion and working with vulnerable populations.

      POSITION REQUIREMENTS (ABILITIES & SKILLS):

      1. Knowledge and experience with implementation of health equity and social care initiatives.
      2. Familiarity with various Health Plan incentive programs including, but not limited to, BCBSM PGIP PCMH, CQ VBR, BCN PRP and PH PIP.
      3. EMR knowledge to understand what documentation is needed to meet Health Plan incentive programs.
      4. Analytical skills to assess data, measure outcomes and recommend mechanisms for improved performance.
      5. Must have a high degree of autonomy and decision-making capacity, independently as well as collaboratively, within a multidisciplinary team.
      6. The ability to identify and recommend creative, cost-effective ways to reduce barriers to implementation of health equity and social care initiatives is required.
      7. Considerable knowledge of health care functions, including clinical functions, medical billing and coding procedures, reimbursement practices and quality improvement initiatives.
      8. Must have the ability to evaluate outcomes of care and effectiveness of health equity and social care initiatives.
      9. Knowledge of the legal and compliance aspects of health information systems and best practices in medical office operations; Knowledgeable about Patient Centered-Medical Homes and Accountable Care Organizations.
      10. Proficient in operating a standard desktop and Windows-based computer system including, but not limited to, electronic medical record software, Microsoft Word and Excel, intranet and computer navigation. Ability to use other software as required while performing the essential functions of the job.
      11. Must have exceptional interpersonal communication skills and the ability to develop effective, collaborative relationships with various levels of organizational members and diverse populations including IHA staff, providers, patients, family members, insurance carriers, vendors and the general public.
      12. Willingness to challenge established ways of doing things in a constructive way.
      13. Strong presentation skills to speak before groups of people, either in-person or virtually, and resolve conflict in situations where the audience may be less than receptive.
      14. Ability to work collaboratively in a team-oriented environment; displays courteous and friendly demeanor.
      15. Good organizational and time management skills to effectively juggle multiple priorities, time constraints and large volumes of work.
      16. Ability to exercise sound judgement and problem-solving skills.
      17. Ability to handle IHA staff and organizational information in a confidential manner.
      18. Ability to travel (10%-25%) to site locations.
      19. Successful completion of IHA competency-based program within introductory and training period.

      5. Specific vision abilities required include close vision, depth perception, peripheral vision and the ability to adjust and focus.
      6. Manual dexterity sufficient to operate a keyboard, photocopier, telephone, calculator and other office equipment.
      7. Requires the ability to drive to other office sites.
      8. Must hear and speak well enough to conduct business over the telephone or face to face for long periods in English.
