- Responsible within an assigned geography to work closely with payor partners, while collaborating with internal stakeholders, to deliver identified enrollment numbers.
- Utilizes internal and external payor data to identify patients who should/should not be included in value-based contracts and works with payors to properly attribute members.
- Identifies new opportunities to grow membership numbers within assigned geography.
- Works closely with internal Population Health, Payor Contracting and Finance teams to ensure meaningful progress to goals while driving for results.
- Works on multiple projects and assignments as a subject matter expert to support leadership priorities, while continuously collaborating with providers and identified patient populations.
- Maintains working knowledge of third-party health care initiatives in which BSMH participates: MSSP, Medicare Advantage and Medicaid.
- Reviews, identifies, and develops strategies for the strategic direction of attribution and performance within BSMH.

Primary Function/General Purpose of Position
This role is responsible for patient attribution and retention within Bon Secours Mercy Health's (BSMH) value-based contracts including Medicare Shared Savings Program (MSSP), Medicare Advantage and Medicaid plans. This role will primarily interact with physicians, providers, and patient populations to deliver enhanced membership and enrollment in value-based contracts while ensuring opportunities to grow and retain populations. This role will serve as the subject matter expert for attribution methodology and Alternative Payment Models while driving for ministry defined Key Performance Indicators (KPIs) and results.

Licensing/Certification
None
Education
Bachelor's Degree in healthcare, business, or related field (required)
Work Experience
5 years in roles within the healthcare field (required)
3 years in patient and physician-facing positions (preferred)
Experience with Medicare Advantage or Medicare Shared Savings plan attribution methodology (preferred)
Working Conditions
Periods of high stress and fluctuating workloads may occur.
Long-distance or air travel as needed
General office environment.
May have periods of constant interruptions.
Required to car travel to off-site locations, occasionally in adverse weather conditions.
Prolonged periods of working alone.
