What to Expect (Job Responsibilities):
- Support efforts to meet quality goals by facilitating preventative care and chronic disease management for primary care patients
- Identify opportunities for workflow changes to improve quality of care related to organizational strategic quality goals
- Manage high volumes of patient outreach for populations with chronic illnesses and preventative screening needs
- Build relationships with patients to assist the primary care team in developing effective care plans
- Navigate data for large patient populations to identify clinical care opportunities and coordinate action items
What is Required (Qualifications):
- High School diploma or equivalency certificate (e.g., GED, HiSET, TASC Test) from an accredited institution
- Minimum of three years of experience in a clinical business support, clinical research, ambulatory care setting, or customer service business
- Proficiency in Microsoft Office and ability to manipulate spreadsheets and utilize data analysis principles
- Strong interpersonal skills and the ability to build productive relationships with patients and healthcare professionals
- Excellent communication skills, both verbal and written, in the English language
How to Stand Out (Preferred Qualifications):
- Associate's Degree or some college coursework, preferably in a healthcare field
- Previous training or experience in population health management
- Knowledge and understanding of medical terminology
- Proficiency in Excel
- Experience with EMR systems or aptitude to master EMR based on proficiency in other technologies
