Job Description

- Delivery of frail elderly assessments, dementia reviews and learning disability health checks
- Identify and manage as appropriate treatment plans for patients at risk of developing a long-term conditions
- Deliver opportunistic health promotion & lifestyle, NHS Health Checks
- Assess and care for patients presenting with minor injuries
- Support and advise in relation to family planning and contraceptive needs
- Deliver cervical cytology screening clinics
- Deliver vaccination and immunisation programmes for both adults and children
- Home visits to the housebound for frail elderly and long term condition reviews as well as delivery of vaccination programmes.
- Administer vaccinations for patients travelling abroad
- Promote use of PGDs
- Participate and lead on QOF & QIF- local quality frameworks & enhanced services.
- Participate in audit
- Participate in practice clinical meetings
- Maintain registration and revalidation processes
- Manage own clinics and have excellent communication and IT skills
- Lead on infection control processes and audit
- Delivery of Cervical Cytology clinics
- Delivery of routine immunisations& Childhood immunisation programme
- Offer a holistic approach to travel health by providing comprehensive advice to patients prior to travel including; vaccinations and medicines, safe sex, food hygiene, sun protection.
- Offer advice about childhood and adult vaccinations and ensure vaccines are administered under patient group directions.
- Running well person clinics and health promotion programmes by providing a holistic assessment and lifestyle advice on diet, smoking, alcohol intake and exercise. Facilitates change.
- Carry out home visits to housebound and frail elderly population for annual reviews, vaccines and long term condition managment
- Assessing problems presented opportunistically by patients, and signposting appropriately.
- Able to document Consultations according to NMC guidelines
- Ensures awareness of statutory and local clinical protection procedures, including systems of referral. Ability to recognise signs and symptoms of child abuse.
- Participation in the running of chronic disease management clinics, learning disability health checks and elderly health checks
- Provide a holistic approach to wound management and implement wound care in line with current evidence based guidelines.
- Taking of swabs
- Contraception
- Able to recognize and manage anaphylaxis according to current UK guidelines.
- Able to perform Cardio-pulmonary resuscitation according to current UK guidelines.
- Ability to obtain and document informed consent (either verbal or written).
- Ensure infection control guidelines are maintained.
- Participation in the delivery of QOF & QIF and other national & local enhanced services.
- Ability to monitor and manage maintenance of stock and equipment to include refrigeration and emergency equipment.
- Confidentially of information gained at work must be preserved at all times.
- Assist patients to identify their health needs.
- Manage and organise individual patient consultations.
- Aware of identification and reporting procedures related to professional standards.
- To attend staff meetings /clinical governance meetings
- Participate in clinical audit

Job summary
Adderley Green Medical Services is looking to recruit an experienced, enthusiastic and highly motivated General Practice Nurse to join our friendly nursing team. The role is flexible and we are open to considering full time or part time - working across our Meir & Longton practice sites
It is requirement that you are experienced in General Practice Nursing and fully competent and trained to take cervical smears and to deliver national immunisation programmes, particularly childhood immunisations and travel vaccines.
Our practice teams are dedicated to delivering high quality, patient-centred care and making a positive difference to patients lives.

The post holder must be experienced & competent in all areas of general practice nursing and you will
Assess, plan, develop, implement, and evaluate programmes to promote health and wellbeing
Deliver and manage long term condition clinics - including child & adult asthma reviews, COPD reviews and diabetic reviews
