What to Expect (Job Responsibilities):
- Provide coordinated physical therapy to patients of all age groups under the supervision of a licensed Physical Therapist
- Administer treatments as directed by the Physical Therapist and record treatments in the medical record
- Maintain clinical documentation according to regulatory, corporate, and professional guidelines
- Participate in weekly case conferences and interdisciplinary communication to ensure patient needs are met
- Provide verbal or written patient status updates to the supervising PT prior to reassessment and discharge visits
What is Required (Qualifications):
- Licensed Physical Therapy Assistant per state requirements
- Minimum of 2 years of experience in Home Health preferred
- Strong communication, assessment, and clinical documentation skills
- Ability to read, write, and speak the English language
- Reliable transportation with valid and current auto liability insurance
How to Stand Out (Preferred Qualifications):
- Experience working with diverse patient populations in home health settings
- Familiarity with electronic medical record systems
- Additional certifications in physical therapy specialties
- Strong organizational and time management skills
- Ability to work independently and as part of a team
