      Position Summary

      Under the direct supervision of the SVP of Behavioral Health Services, the Early Childhood Development (ECD) Project Manager is responsible for supporting the Early Childhood Development program by being a key member of the care team. The ECD program focuses on children ages 0-5 and their families. The Project Manager will support the program by coordinating trainings, achieving milestones and project deliverables by ensuring data integrity of clinical indicators entered in eClinical Works, and preparation of data analysis of project reports for funders, grants, and studies. The Project Manager will support the SVP of BH, VP of Quality Assurance and Clinical Risk Management, and other management staff with analysis of data and processes, and preparing documents for process flow charts, presentations, or policies. The Project Manager must be a critical thinker and analyze information, have strong attention to detail, organized, self-motivated, able to work independently and interact well with individuals and in groups, work collaboratively, meet project deadlines, and possess strong problem-solving skills.

      Essential Functions, Duties, and Responsibilities

      • Be familiar with Healthy Steps model
      • ECD Program Team will undergo the Healthy Steps onboarding process
      • Participate in monthly trainings conducted by Healthy Steps
      • Attend professional development opportunities such as training opportunities provided through Healthy Steps and ZERO TO THREE
      • Hold regular meetings with ECD providers, ECD Specialist and other staff to achieve deliverables according to the workplan and to communicate regularly regarding patient progress
      • Utilize database and EMR systems to stratify children by risk following the Healthy Steps model in collaboration with the program staff
      • Identify and assist with stratification so that all children ages 0 to 5 and their caregivers served at Zufall’s Dover and West Orange sites will receive Tier 1 services, which include developmental, socio-emotional, and behavioral screenings; family needs screenings; and social determinants of health screenings
      • Assist with patient and family stratification into Tier 2 (for those requiring short-term support) or Tier 3 (for those requiring long-term support) groups using data reports and screening results to assist the implementation team
      • Collaborate with the ECD Specialist to develop assessments and processes to serve Tier 2 and Tier 3 families
      • Work with IT and QA to develop reports and data entry workflows so that all screenings performed using standardized tools are available to the program staff and that the data is accurately reported
      • Using data reports, identify those that have not been screened and those whose score indicates the need for additional screenings and additional services.
      • Utilize existing screening tools in ECW such as PRAPARE and others to create workflows and documentation for the ECD Providers and Specialist and on reports for identification of most prevalent barriers in our populations
      • Prepares routine reports documenting results of findings
      • Creates reports, tables and graphs using Excel, Word, or PowerPoint
      • Participate in formal and informal presentation of findings in task forces, subcommittees, and committee meetings
      • Support department and program management staff by preparing reports and monitoring performance measures in funded programs
      • Prepares appropriate modifications to eCW to improve data integrity, process flows and reports
      • Assists with program reporting requirements and current data collection systems in order to ensure efficiency
      • Design source and output documents
      • Assists with training for staff members and providers when new procedures are implemented
      • Assists with developing policies and procedures for the ECD program
      • Extract and analyze data for program development and planning purposes
      • Assists with preparation of monthly and annual data reports for review by the SVP of Behavioral Health Services and other managers
      • Perform other duties as assigned

      Knowledge, Skills, and Abilities

      • Clinical background in community health (ex. case management, social work or nursing) preferred
      • Strong project management skills
      • Learn and apply the trauma informed care principles within the scope of the position
      • Bilingual Spanish preferred
      • Some travel between sites may be required

      Education, Training, and Experience

      • Bachelor's degree required
      • Experience working with community-based organizations as support staff, evaluator, direct service provider or consultant preferred
      • Experience working in an interdisciplinary, team-based model
      • Experience with data entry and report creation, database development, data management and analysis preferred
      • Must be proficient in Excel, Word, PowerPoint. Experience with electronic health records preferred (ex. eCW)
      • Experience providing training and/or technical assistance
