What to Expect (Job Responsibilities):
- Provide coordinated physical therapy to patients as directed by the Physical Therapist
- Maintain accurate records of treatments administered and clinical documentation according to regulatory and professional guidelines
- Participate in weekly case conferences and interdisciplinary communication to optimize patient care
- Provide verbal or written updates on patient status to supervising PT prior to reassessment and discharge visits
- Ensure patient safety and compliance with state regulations and national standards in physical therapy practice
What is Required (Qualifications):
- Licensed Physical Therapy Assistant per state requirements
- Minimum of 2 years of experience in Home Health preferred
- Strong communication, assessment, and clinical documentation skills
- Ability to read, write, and speak the English language
- Reliable transportation with valid and current auto liability insurance
