The Remote Patient Monitoring (RPM) Home Telehealth (HT) Nurse Case Manager executes position responsibilities that demonstrate leadership, experience, and creative approaches to the management of patient care. RPM-HT is a program that coordinates care using health informatics, disease management and home telehealth technologies to facilitate access to care and improve the health of Veterans with the intent of providing care, in the right place, at the right time.
