What to Expect (Job Responsibilities):
- Serve as the primary contact for patients and families transitioning to or from post-acute services
- Make clinical level-of-care determinations using medical records and clinical data
- Provide education and guidance on home health, hospice, and palliative care options
- Advocate for patients throughout the referral process to ensure timely responses
- Track referral outcomes to ensure high-quality care transitions
What is Required (Qualifications):
- Associate degree required; Bachelor’s degree preferred
- Minimum of 3 years of post-acute care experience
- Active professional licensure as a Registered Nurse (preferred) or LPN in a compact state
- Strong knowledge of home health, hospice, and palliative care services
- Excellent communication and customer service skills
How to Stand Out (Preferred Qualifications):
- Experience in home health and hospice settings
- Proficiency in Microsoft Office Suite and strong technology skills
- Additional certifications related to post-acute care
