What to Expect (Job Responsibilities):
- Utilize clinical expertise to manage patients' transition from acute care setting to home setting through telephonic outreach
- Coordinate all services required for a patient to transition home safely
- Provide education, coaching, and care coordination to accomplish program goals
- Manage length of stay and reduce hospital readmissions
- Outreach to patients prior to hospitalization or discharge
What is Required (Qualifications):
- 6 years of Nursing Experience as a Registered Nurse
- Registered Nurse with a current and unrestricted license for the market of practice
- Excellent communication, problem-solving, and decision-making skills
- Demonstrated coachability or history of learning new Electronic Medical Record systems
How to Stand Out (Preferred Qualifications):
- Experience in Case Management
- Strong experience in healthcare coaching
- Multi-state/compact license
