Remote Patient Monitoring Nurse (Hybrid) - Healthcare Technology Company
Located in Richmond, VA
Salary $40 to $50 Per Hour (based on experience)
A growing healthcare technology company is seeking an experienced RN or LPN for a Remote Patient Monitoring role.Hybrid position: mix of remote telehealth work and 1–2 on-site days at a local partner facility in Richmond.
Qualifications:
• Active VA RN or LPN/LVN license (good standing).
• 2+ years of experience in geriatrics, home health, ALF/SNF, or telehealth.
• Prior Remote Patient Monitoring (RPM) or Chronic Care Management (CCM) experience (CMS codes 99453–99458).
• Tech-savvy – experience with EHRs, Bluetooth medical devices, and documentation.
• Strong communication skills and ability to educate patients and caregivers.
• Valid driver’s license must be local to Richmond, VA.
Preferred:
• Background in chronic disease management (HTN, CHF, COPD, CKD, diabetes).
• BLS certification; Bilingual Spanish speaking
