Job summary
The Role
We require the candidate to have at least 1 years experience in primary care and have completed their CPPE Pathway.
To undertake medicine reconciliations of patient record/prescribing in line with the practice protocols. Update patient records, support medicine reviews and synchronization for groups of patients i.e., care homes and help patients to get the best from their medicines by switching the medications to agreed and approved protocols. You will carry out prescribing audits and medicines safety alerts.
In recognition of the important role pharmacy technicians can play in general practice we are seeking a professional who can support patients to get the most from their medicines, contributing to Structures Medication Reviews (SMRs) in collaboration with the clinical pharmacist and for their skills and knowledge around acute and repeat prescribing processes and improving patients safe use of medicines to contribute toward the management of patients on site.
