      What to Expect (Job Responsibilities):
      - Assist patients in arranging transportation to/from the hospital for ongoing medical treatment
      - Maintain files of brochures, information packets, and applications for community resources
      - Collaborate with social work staff and community agencies to obtain necessary information
      - Educate patients and families about available resources and how to access them
      - Assess patient and family understanding of available community resources

      What is Required (Qualifications):
      - High School Diploma or Equivalent required; Associate's Degree in a related field preferred
      - 2-3 years of related experience preferred
      - Familiarity with medical terminology and community services/resources
      - Strong interpersonal, customer service, and advocacy skills
      - Excellent written and verbal communication skills

      - Experience working in a fast-paced healthcare environment
      - Knowledge of current community resources and crisis intervention techniques
      - Proven ability to maintain effective working relationships with patients and families
