      What to Expect (Job Responsibilities):
      - Assist patients in arranging transportation to/from the hospital for ongoing medical treatment
      - Collaborate with unit-based case managers to facilitate discharge plans to psychiatric or medical settings
      - Research and maintain information on community social resources
      - Educate patients and families about available resources and how to access them
      - Organize and disseminate information about community resources, including referrals and in-service presentations

      What is Required (Qualifications):
      - High School Diploma or Equivalent required, Associate's Degree in a related field preferred
      - 2-3 years of related experience preferred
      - Familiarity with medical terminology and community services/resources
      - Strong assessment, crisis intervention, and interpersonal skills
      - Excellent written and verbal communication skills

      - Experience working with social work staff and community agencies
      - Proven collaborative skills in a fast-paced environment
      - Ability to maintain effective working relationships with patients and families
      - Knowledge of current community resources and advocacy skills
