What to Expect (Job Responsibilities):
- Manage a caseload of patients with diabetes through telephonic and/or virtual means
- Assess patient needs and develop individualized care plans
- Utilize technology to interact with patients and analyze data
- Collaborate with interprofessional teams to ensure quality care delivery
- Serve as an expert resource within the department for diabetes care
What is Required (Qualifications):
- Registered Nurse license issued by the state of IL and/or WI
- Bachelor's Degree in Nursing
- Registered Certified Diabetes Educator (C.D.E.) or eligibility to take Board exam
- 5 years of experience in patient care as a certified diabetes educator
- Proficiency in Microsoft Office and previous Epic EMR experience
How to Stand Out (Preferred Qualifications):
- 0-2 years of experience as an Rx Technician or Customer Service Representative in a healthcare-related service
