      Horizon Health is a Critical Access, Rural Health Facility comprised of 25-inpatient beds located in Paris, IL & a multitude of outpatient clinic settings including Family Practice and Specialty Clinics in Paris and surrounding cities. We have been serving residents of Edgar County since 1968 though community education, emergency services, and outpatient care.       Position Summary:

      The EZ Care/NAL Clinic nurse lead will work directly with clinical staff and the EZ Care/NAL manager to ensure that clinic patients receive high quality care while demonstrating the organization’s mission, vision, and values.

      Essential Functions:

      • Assists with the flow of patients throughout the clinic.
      • Documents all patient and telephone transactions concisely, descriptively, accurately, legibly and completely.
      • Provides quality customer service by being helpful, friendly, and professional with patients, providers, visitors, and employees.
      • Demonstrates ability to analyze situations and arrive at sound, practical decisions using good clinical judgment.
      • Acts as a patient advocate, facilitates patients into the system, and help patients attain what he/she needs.
      • Facilitates communication with staff and nursing manager; performs huddles. Participates in staff meetings with manager and conducts annual re-education/annual competencies.
      • Participates in committee work including potentially leading/facilitating department committees as assigned.
      • Assists with special projects as assigned including but not limited to new employee orientation; implementation of new service lines and working with quality nurse and nursing manager to perform documentation review.
      • Fields and addresses patient and staff concerns.
      • Assists with reviewing/revising patient care policies.
      • Assists with requisitioning and inventory of equipment and supplies.
      • Assists with implementation and ongoing monitoring of standards and quality assurance activities; refrigerator temps, device certification, etc.
      • Assists with staff scheduling and staffing coverage as needed.
      • Through leadership and example assists with enforcement of policies.
      • Provides direct patient care in high demand times.
      • Assists with maintaining up to date employee licensing and certifications.
      • Offers input and insight on staff evaluations.
      • Shifts will include occasional weekends.

      The responsibilities listed above are not all-inclusive; other activities may be required in support of the hospital’s goals and objectives.

      Position Requirements:

      • Graduate of accredited school of nursing
      • Current Illinois license as LPN or RN
      • Current CPR certification
      • Ability to communicate well both verbally and in writing
      • Knowledge of nursing process and use of the same
      • Good nursing practice skills
      • Ability to perform complex and varied tasks in a fast-paced work environment subject to stress and interruption
      • Diplomacy in dealing with sensitive issues
      • Experience with conflict resolution

      Position Information:

      Location: 1 Phipps Lane Paris, IL 61944

      Hours: Mainly Monday-Friday 830am-5pm. Has the potential to be asked to pick up outside out working ours. One weekend shift a month.
