What to Expect (Job Responsibilities):
- Monitor the CareSignal patient care dashboard to manage notifications and alerts for enrolled patients
- Provide direct care using CareSignal data for a manageable volume of patients
- Collaborate with Clinical Services and ACO stakeholders to enhance the quality of care
- Follow standard operating procedures (SOPs) to deliver effective patient care
- Identify and outreach to specific patient populations eligible for health-related services
What is Required (Qualifications):
- Minimum of 2 years post-degree experience in a direct healthcare setting
- Active RN or LPN license in the state of Illinois
- Proficient in interpreting clinical treatment information and making triage decisions
- Skilled in patient education on various healthcare-related issues
- Comfortable communicating with patients in-person, over the phone, and virtually
How to Stand Out (Preferred Qualifications):
- Previous experience in Care Coordination, Case Management, Home Health, or Behavioral Health
- Two years of experience in a provider practice or clinic health setting
- Familiarity with health IT systems and modern EHR management
- Experience managing patients with chronic conditions
- Knowledge of population health programs and resources
