What to Expect (Job Responsibilities):
- Provide case management services to HIV-infected individuals, including assessment and enrollment in needed services
- Collaborate with healthcare teams to develop and implement case management plans
- Communicate effectively with clients and coordinate care through face-to-face and telephonic interactions
- Document all care coordination activities in individual client records
- Participate in performance improvement activities and cover for other case managers as needed
What is Required (Qualifications):
- Master's degree in Social Work
- Licensed Master Social Worker (LMSW) - State Division of Professional Registration upon hire
- Knowledge of health and social services available to individuals with HIV
- Ability to communicate effectively with clients and healthcare teams
- Commitment to following policies, procedures, and compliance standards
How to Stand Out (Preferred Qualifications):
- Experience in case management or working with HIV-infected populations
- Familiarity with grant preparation and reporting processes
- Participation in community health initiatives or partnerships
