      We are looking for an experienced GP to take up this exciting new leadership role. The role will involve working as Clinical Lead for two sessions a week in a managerial role and 4-6 clinical sessions at Guiseley and Yeadon Medical Practice leading the clinical team daily there. We are looking for a candidate who can work Tuesdays, Thursdays and Fridays.

      We have a sister practice at East Park Medical Centre in Leeds that also holds several Intermediate Care Contracts. This role will work alongside other leads for these services and under the supervision of the senior management team who oversee all the services.

      The Clinical Lead will provide clinical supervision and support training of the multi-disciplinary team. They will also carry out clinical governance duties supported by our Clinical Governance and Quality Improvement Lead. They will report to the Clinical Director and GP Partner.

      The Clinical Lead will work to support innovative changes introduced to help deliver Primary Care services across both practices. These include working to improve health inequalities in our population and closely with our PCN services.

      The Clinical Lead will work with the team to ensure CQC standards are maintained.

      The post holder will work sessions as the Clinical Lead at Guiseley and Yeadon Medical Practice

      Providing medical leadership for the delivery of high quality, safe, and reliable medical services

      Carrying out clinical governance activities including clinical audits, review of significant events (datixes) and clinical complaints with route course analysis, chairing 2 weekly clinical meetings, carrying out appraisals for clinical team, supporting safeguarding alongside safeguarding leads for services and support quality improvement programs and outcomes.

      To work within a new Tier Based Care Delivery Model which helps support receptionists with safe signposting and to identify the most vulnerable patients.

      To support the diversity of the current MDT model in Primary care and integrate the roles into a safe and effective service.

      Leading a multidisciplinary team of clinicians and debriefing them after consultations, reviewing management plans and prescribing

      Role Specific Responsibilities Executive Director with responsibility for medical performance:

      To ensure that all doctors and pharmacists advanced nurse practitioners, mental health practitioners and physician associates undertake all required professional development and adhere to professional standards.

      Lead the management of any team or individual to maximise their contribution and, where appropriate, with regard to conduct/performance concerns, ensure appropriate action is taken to provide and support development and manage performance.

      Lead on ensuring all doctors demonstrate attitudes and behaviours that are congruent with practices vision, values, clinical and GMP standards.

      Promote a culture of leadership in the practice in order to ensure medical staff engage in the achievement of the Companys objectives.

      Lead on ensuring that the practice achieves consistently high clinical outcomes and a positive patient experience through effective audit, implementation of best practice and ensuring clinicians recognise patient perceptions and feedback.

      Oversee Clinical governance activities including audits, SEAs, clinical complaints, appraisals and escalating safeguarding concerns.

      Collaborate with other members of the leadership team to ensure QOF and other income generating targets are achieved by practices and services.

      Collaborate with other members of the leadership team and senior managers to develop multi professional working including medical workforce redesign.

      Maintain effective relationships with stakeholders facilitate delivery of care and safe clinical practice.

      Ensure that the safe implementation of new or novel clinical practices or techniques.

      Clinical Sessions:

      To do an agreed number of clinical sessions. Providing the full range of general medical services based on patient needs where appropriate, in accordance with practice opening times and Enhanced Service Contracts and any other such services as should arise from time to time.

      In accordance with the Practice timetable, as agreed, the post-holder will make themselves available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork, reports and correspondence in a timely fashion.

      Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.

      Assessing the health care needs of patients with undifferentiated and undiagnosed problems.

      Screening patients for disease risk factors and early signs of illness.

      In consultation with patients and in line with current Practice disease management protocols, developing care plans for health.

      Providing counselling and health education.

      Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate.

      Recording clear and contemporaneous consultation notes to agreed standards.

      Collecting data for audit purposes and complying with QOF requirements.

      Delivering enhanced services.

      Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible).

      Prescribing in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriate
