**What to Expect (Job Responsibilities):**
- Conduct in-home assessments three days a week and work remotely two days a week for Alignment members, including medication review and health screenings
- Provide patient education by aligning desired care with the best available options
- Identify diagnoses for assessment in care management and active medical management
- Collaborate with the Regional Medical Officer and team members to develop appropriate care plans based on patient needs
- Coordinate care with multiple stakeholders, including primary care physicians, specialists, and other providers

**What is Required (Qualifications):**
- Must possess an active Nurse Practitioner or Physician Assistant license
- Master's degree from an accredited NP or PA program
- Ability to communicate positively and effectively with others
- Strong problem-solving, organizational, and time management skills
- Valid Driver's License and current automobile insurance

**How to Stand Out (Preferred Qualifications):**
- One (1) year of prior clinical or home care experience
- Previous experience with Electronic Medical Records (EMR)
- Experience in the care of older adult (geriatric) patients
- Active RN License and Furnishing number (for NP only)
- Active Nurse Practitioner Board Certification/Physician Assistant Certification
