What to Expect (Job Responsibilities):
- Lead the implementation and execution of clinical pharmacy services across multiple practice sites
- Deliver direct patient care through comprehensive medication management and chronic disease state management
- Develop treatment algorithms and drug utilization management strategies to enhance patient care
- Communicate effectively with the healthcare provider team to ensure aligned patient care
- Manage referrals and address patient therapy-related inquiries and issues

What is Required (Qualifications):
- Doctorate of Pharmacy from an accredited college or school of pharmacy
- Current active pharmacist licensure and in good standing
- Completion of a PGY 1 and/or PGY 2 AmbCare residency or at least 4 years of disease state management experience
- Knowledge of chronic disease states such as diabetes and cardiovascular disease
- Willingness to travel up to 10% of the time for business purposes

How to Stand Out (Preferred Qualifications):
- Bilingual in English and Spanish
- Multistate licensure or willingness to obtain other state pharmacy licenses
- Completion of a PGY1 and PGY2 AmbCare residency with over 5 years of AmbCare experience
- Board Certified in relevant pharmacy specialties
- At least 1 year of leadership experience in a healthcare setting
