      What to Expect (Job Responsibilities):
      - Work with a Clinical Care Team to support patient and provider engagement
      - Engage high-risk patients telephonically to schedule appointments with Clinical Care Team
      - Utilize multiple technology systems and outreach to capture pertinent information for patient engagement
      - Support patients by scheduling PCP appointments on their behalf
      - Facilitate exchange of clinical information between Care Team and provider offices

      What is Required (Qualifications):
      - High School Diploma / GED
      - Must be 18 years of age or older
      - 2+ years of Prior Medical / Healthcare experience
      - 2+ years of experience working with medical terminology
      - Computer proficiency with Microsoft Outlook

      How to Stand Out (Preferred Qualifications):
      - Pharmacy Tech, Pharmacy Tech for Insurance Company, or Medical Assistant experience
      - Managed Care Experience
      - Bilingual fluency in English and Spanish
