      Job summary

      This is a new and exciting service for Norfolk Community Health and Care. As a Community Intravenous Nurse you will be delivering a bespoke and intricate service in peoples own dwellings, allowing them to continue hospital prescribed treatment at home, thus shortening their time in hospital.We are looking for dynamic and patient focused nurses who want to advance their clinical skills.

      You will liaise directly with team members across the acute and community services and be responsible for monitoring all aspects of the patients response to their treatment and feeding back to the acute trust when required and seeking support from the clinical lead when needed.Extensive training and support will be provided once in post, however previous IV experience desirable.

      Main duties of the job

      Your work with the community IV team will involve visiting patients in their own homes and occasionally nursing homes/community inpatient settings. You will be responsible for managing your own caseload, visiting patients at their scheduled time, assessing/maintaining their vascular access device, sampling and monitoring blood reports and safe delivery of the prescribed IV therapy. You will also be responsible for replacing IV access (within your competency skill-set) and performing routine investigations such as observations/NEWS scoring.

      You will be expected to cover the senior nurse role at the Norfolk and Norwich University Hospital (NNUH), receiving referrals, assessing patients in a ward setting and implementing plans to facilitate a safe successful discharge home.

      You will be expected to discuss any concerns with the IV clinical lead, clear lines of escalation will give you easy access to clinical advice and a management plan as and when needed.

      Clinical

      The post holder will need to:

      • Assess, plan, implement and evaluate all aspects of patient care, enabling the safe administration of IV medication in peoples own homes.
      • Following a holistic assessment, devise an individualised, evidence-based care programme for each patient, modifying it as required.
      • Assess patients in the acute trust (Norfolk and Norwich University Hospital) reviewing referrals and accepting suitable patients onto the IV service caseload to facilitate discharge
      • Assess patients holistic needs, communicating complex and sensitive information to patients and carers as to their assessment, diagnosis, prognosis and treatment plan.
      • Monitor and review blood reporting, escalating and implementing relevant actions
      • Where there are barriers to understanding, such as hearing impairment, mental capacity impairment and other difficulties in comprehension, explanations require adjustments in order to gain understanding, consent and concordance.
      • Be required to use tact and persuasive skills in order to gain the patients co-operation in their care management plan.
      • Liaise with the Clinical Lead and Norfolk and Norwich University Trust clinicians when clinical concerns are raised.
      • Liaise with Homeward, Supportive Care, GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated appropriately.
      • Be aware of assistive technology and utilise where appropriate.
      • To participate in multi-disciplinary/multi-agency meetings as appropriate, e.g. Gold Standard Framework.
      • Ensure own caseload is planned and prioritised according to service targets and patient need.
      • Ensure patient held records are completed for each visit, and that SystmOne inputting is completed on a daily basis.
      • Demonstrate dexterity and co-ordination when using specialist equipment and fine tools, advanced sensory skills, manual and mobilising skills.
      • Be skilled at intravenous cannulation, IV line management and general phlebotomy, following local policy and guidance
      • Contribute to the requisitioning of supplies and equipment through electronic ordering systems.
      • Ensure informed consent is obtained prior to initiating interventions.
      • Will be exposed to bodily fluids, infected material, blood products on a daily basis, therefore must utilise universal precautions and adhere to infection control policies.
